Facility Evaluation Report

On 7/10/2025 at 9:45am, Licensing Program Analyst (LPA) Y.Brown conducted an unannounced annual required inspection. LPA met with Program Manager, Teanna Jerkins and explained the purpose of the visit.

The facility’s fire clearance was approved for forty-five (45) clients, all may be ambulatory. The day program operates from 9:30 AM to 3:30 PM. There were 2 (two) clients that participate via Zoom. LPA toured facility with Teanna including but not limited to, multiple activity rooms, kitchen, bathrooms, office spaces, and the outside recreational area. Clients bring their own lunches and snacks but LPA observed that there are emergency supplies, water and snacks available for clients. All indoor passageways are kept free of obstruction. There are no bodies of water. A comfortable temperature for clients is maintained at 76 degrees Fahrenheit. LPAs observed lighting in all rooms are adequate for the comfort and safety of the clients. Hot water temperature in the shared clients’ restroom was measured at 112.2 degrees Fahrenheit.

All toilets, hand washing, and bathing are safe, sanitary and in operating condition. The supply of extra hygiene was available for residents. Smoke and carbon monoxide combination were observed during visit. Fire extinguisher last serviced on 08/19/2024. The Fire Drill was reviewed to be last conducted on 6/27/2025. The emergency disaster plan was reviewed and observed to be last reviewed on 1/12/2025.
