Facility Evaluation Report

On 07/20/2026 at 3:15 PM, Licensing Program Analyst (LPA) L. Alexander arrived unannounced to conduct a Case Management visit. LPA met with Administrator, Evangeline Lekse, and explained the purpose of the visit.

While conducting a complaint investigation, #15-AS-20260713081309 , on 07/20/2026, LPA reviewed resident records and observed that Resident 1's (R1) file was incomplete. Specifically, the file did not contain a current Appraisal Needs and Services Plan or a hospice care plan. LPA requested copies of the missing documents from the Administrator.

Based on record review, deficiencies were observed and cited pursuant to California Code of Regulations, Title 22 (see LIC 809D). Failure to correct the cited deficiencies may result in civil penalties.

An exit interview was conducted with the Administrator. A copy of this report and appeal rights were provided.
