On 08/27/2025, at 6:45 PM, Licensing Program Analyst (LPA) L. Alexander arrived unannounced to conduct a Case Management visit regarding. LPA met with Program Director, Michele Sheldon, and explained the reason for the visit.
LPA requested documents for Resident (R) R1-R5 via email on 08/25/25. On 08/27/25, Administrator called LPA that they had the documents ready which was over 150 pages. LPA agreed to come and pick up documents. Administrator also submitted documents for a 30-Day eviction for R6.
Exit interview conducted and a copy of this report was provided.
