Facility Evaluation Report
On 08/04/2025, at 5:00 PM, Licensing Program Analyst (LPA) L. Alexander arrived unannounced to conduct a Case Management visit regarding an incident that was reported to Community Care Licensing Division (CCLD) on 11/20/2024. LPA met with Campus Administrator, Michele Sheldon, and explained the reason for the visit.
The incident occurred on 11/19/2024 when staff (S1) observed that they incorrectly read the medication card and administer the incorrect medication to Client (C1). The Unusual Incident Report (LIC624) indicated that when S1 discovered that they had given the incorrect medication they contacted the advice nurse.
On 08/04/2025 LPA interviewed S2 that stated there was a Supportive Conversations About Medication training on 11/12/2024. In addition, LPA reviewed the medication training completed for S1.
Exit interview conducted and a copy of this report was provided.
