Facility Evaluation Report
On 01/17/2025, at 11:00 am, Licensing Program Analyst (LPA) L. Alexander arrived unannounced to conduct a Case Management visit regarding an incident that was reported to Community Care Licensing Division (CCLD) on 01/06/2025. LPA met with Campus Administrator, John Carswell, and explained the reason for the visit.
The incident occurred on the evening of 01/04/2025 when staff observed that Client (C) C1 did not return for curfew. The Unusual Incident Report (LIC624) indicated that approximately at 11:00 pm, C1, was reported as a missing person with Pleasant Hill Police Department (PHPD).
LPA interviewed Staff (S) S1. S1 stated that C1 was seen by S2 at a nearby store and that S2 called the on-call supervisor to report. S1 stated that S2 called PHPD and a missing person's report was filed. S1 stated that C1 was found on 01/05/2025 and transported to Contra Costa Regional Medical Center for further evaluation. S1 stated that C1 was admitted to the hospital for treatment and was discharged back to the facility on 01/13/2025. S1 stated that they are planning an Interdisciplinary-Team (IDT) meeting to go over and address the care plan for C1. LPA reviewed C1's physician's report, dated 08/24/2023, that reveals C1 can leave the facility unassisted.
