Facility Evaluation Report
On 07/01/2026 at 9:45 AM, Licensing Program Analyst (LPA) L. Alexander arrived unannounced to conduct 1-Year Annual Required inspection. LPA met with Business Office Manager, Vanessa Gomez, and explained the purpose of the visit. Campus Administrator, Michele Sheldon, arrived approximately 10:30 AM. The facility’s fire clearance was approved for capacity of sixty-four (64) clients. Age range 18 through 59, all ambulatory. Administrator Certificate #7025403735 expires 05/31/2027.
LPA toured the facility including but not limited to bedrooms, bathrooms, kitchen, common areas. The facility consists of 32 total bedrooms which 32 bedrooms are occupied by the clients. All outdoor and indoor passageways are kept free of obstruction. There are no bodies of water. A comfortable temperature for clients is maintained 70, 73 degrees Fahrenheit. LPA observed lighting in all rooms are adequate for the comfort and safety of the clients. Hot water temperature in the shared clients’ bathroom was measured at 105, 110.7 degrees Fahrenheit. All toilets, hand washing, and bathing are safe, sanitary and in operating condition. The supply of extra hygiene’s were available for clients. There is a minimum of one week supply of non-perishable and two-day (2) perishables food supply.
