Facility Evaluation Report
On 01/17/2025, at 10:30 am, Licensing Program Analyst (LPA) L. Alexander arrived unannounced to conduct a Case Management visit. LPA met with Campus Administrator, John Carswell, and explained the reason for the visit.
While LPA L. Alexander was conducting a complaint investigation (15-AS-20240926145838) on 01/17/2025. During the investigation, LPA obtained documents for residents' files. During record review, LPA observed that residents that are 60 years and older were missing, including but not limited to, updated medical assessments, updated appraisal needs and services, and documentation that shows that the Adult Residential Facility (ARF) is an appropriate setting for them.
The deficiencies were observed (see LIC809D) and cited from the California Code of Regulation, Title 22. Failure to correct the deficiencies may result in civil penalties.
