Facility Evaluation Report
On 10/14/2025, at 12:30pm, Licensing Program Analyst (LPA) L. Hall conducted an unannounced Case Management visit regarding an incident that was reported to CCLD on 10/09/2025. LPA met with Warren Garibay, Direct Support Staff, and explained the purpose of the visit. Administrator, Christopher Jo, arrived at 1:12pm.
The incident occurred on 10/09/2025, and stated C1 fell out of wheelchair and broke her femur. During the visit LPA interviewed S2. S2 stated there were two (2) staff and four (4) clients present. C1 was in her wheelchair with the wheels locked. S2 stated she helped C2 to the van to attend the day program, came back in and went to get clothes for C1. During the time S2 was getting C1's clothing she heard a loud noise and observed C1 turned over. S3 was nearby but not observing C1.
LPA obtained a copy of the individual program plan (IPP), client roster, and staff schedule during visit.
*A civil penalty of $500.00 will be assessed on today's date for regulation 85065(a)(b).
The deficiency was observed (see LIC809D) and cited from the California Code of Regulation, Title 22. Failure to correct the deficiencies may result in civil penalties.
