The inspector’s account

On 4/23/2025, at 2:50pm, Licensing Program Analyst (LPA) L. Hall arrived unannounced to conduct a proof of correction (POC) visit. LPA met with Jenna Asuncion, Direct Care Staff and explained the purpose of the visit. Administrator, Maria Matel, arrived at 3:25pm.

LPA conducted an annual visit on 4/9/2025 and cited for the following deficiency that has been corrected.

80088(h), LPA observed both patio doors now have screens.

80088(e)(1), LPA observed hot water temperature has been adjusted to 129.9.

80026(h), LPA observed R1, R2, and R3 cash records were updated.

80077.3(j), LPA observed facility conducting fire drill during visit on 4/23/2025.

80069(b)(1), LPA observed Administrator have either scheduled appointment or email regarding assessment.
