On 5/12/2026, at 12:00pm, Licensing Program Analyst (LPA) L. Hall arrived unannounced to conduct a proof of correction (POC) visit. LPA met with Jenna Asuncion, Caregiver, and explained the purpose of the visit. LPA spoke with Administrator Maria Matel, via telephone.

LPA conducted an annual inspection on 4/28/2026 and cited for the following deficiencies that has not been corrected.

87463(a) - LPA observed service plans were not completed.

87217(g) - was cleared during initial inspection on 4/28/2026.
