The inspector’s account

On 04/28/2026 at 2:35pm, Licensing Program Analyst LPA L. Hall conducted an unannounced annual required inspection. LPA met with Caregiver, Jenna Asuncion. Administrator, Maria Matel, arrived at 3:20pm, and explained the reason for the visit. The administrator currently holds a certificate (#7033531740) that expires on 01/29/2027. The facility’s fire clearance was approved for six (6) Non-ambulatory and one hospice waiver for three (3).

LPA toured the facility including but not limited to bedrooms, bathrooms, kitchen, common area and backyard. The facility consists of six (6) bedrooms and two (2) bathroom. Two (2) bedrooms are occupied by staff. No bodies of water observed. LPA observed lighting in all rooms are adequate for the comfort and safety of the residents. A comfortable temperature is maintained at 74 degrees Fahrenheit. LPA observed lighting in all rooms are adequate for the comfort and safety of the residents. The hot water temperature in the residents’ shared bathroom was measured at 106.5 degrees Fahrenheit. Night lights are maintained in hallways and passages.
