This is an official report of an unannounced visit/investigation of a complaint received in our office on

09/29/2021 and conducted by Evaluator Lizette Francisco

COMPLAINT CONTROL NUMBER: 15-AS-20210929091703

Client was physically abused while in care.

On 1/6/2022 starting at 10:00 AM, Licensing Program Analysts (LPAs) L. Francisco and G. Luk arrived unannounced to deliver findings for the above allegation. LPAs were greeted by Caregiver, Shamari Washington-Pitre and informed the purpose of the visit.

During the course of the investigation, LPA L. Francisco obtained information, collected documents, interviewed 7 staff and 2 clients. It was alleged client was physically abused while in care. However, based on interview, 6 of 7 staff and 2 of 2 clients denied allegation.

Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is UNSUBSTANTIATED.
