Facility Evaluation Report

07/08/2025, at 3:30pm, Licensing Program Analyst (LPA) L. Hall conducted an unannounced Case Management visit regarding an incident that was reported to CCLD on 7/3/2025. LPA met with Patricia Vasquez, Direct Support Staff. Chris Chavez, Lead Staff arrived at 3:17pm, and explained the purpose of the visit. LPA spoke with Administrator, Cynthia Chavez, via telephone.

The incident occurred on 7/2/2025, where C1 AWOL'd from facility. S1 stated that C1 was upset regarding a family issue, S1 heard the sensor of the front door go off, and noticed that C1 had left the facility. C1 was found approximately 1-hour later by S1. S1 stated C1 has Crisis Response Personnel (CRP) that he speaks with and on 7/18/2025, there will be a meeting held with behaviorist to help assist C1, and S1 requested full communication from the family to assist C1 with feelings.
