Facility Evaluation Report
On 4/12/2023 at 12:05PM Licensing Program Analyst (LPA) L. Hall arrived unannounced to conduct a Case Management visit. LPA met with Rebecca Lapasa, Administrator.
While LPA L. Hall was conducting a complaint investigation (15-AS-20230405110437 ) on 4/12/2023, during interview with staff and review of complaint documents LPA observed C1 was hospitalized on 3/30/2023. Staff did not report or submit an incident report to CCLD stating the information.
The deficiency was observed (see LIC809D) and cited from the California Code of Regulation, Title 22. Failure to correct the deficiency and/or repeat deficiency in a 12-month period may result in civil penalties.
