Facility Evaluation Report

On 5/24/2024, at 3:50PM, Licensing Program Analyst (LPA) L. Hall arrived unannounced to conduct proof of correction (POC) visit. LPA met with Quent Lapasa, Caregiver, and explained the purpose of the visit.

Facility cleared the following deficiency upon visit.

80020(c) - Fire drill was last conducted on 12/20/2023. Facility will continue to conduct fire drills.

LPA approved extension for the following deficiencies until 6/7/2024.

Exit interview conducted. and a copy of this report provided.
