LPA also toured the bedrooms and bathroom during visit. LPA did observe a laundry basket full of clothing in C2's room. LPA did not observe any toiletries in bathroom for clients. Staff showed LPA toiletries that were locked up in the staff room, which included shampoo, soap, and deodorant. C3 stated during interview that he buys his own toiletries and keeps them in his room along with his towels. C3 conducts his own activities of daily living (ADL's). During interview with C4, he showed LPA his travel sized shaving case with his toiletries. C4 stated he conducts his own ADL's. LPA observed C1 come and out room and leave facility. LPA observed C1 in unsanitary clothing and unkempt grooming. LPA did not interview C1.
Based on LPA's observations and interviews which were conducted and record reviews, the preponderance of evidence standard has been met, therefore the above allegation is found to be SUBSTANTIATED . California Code of Regulations (Title 22, Division 6, Chapter 8), are being cited on the attached LIC 9099D.
