Facility Evaluation Report

On 1/29/2026 at 4:00pm, Licensing Program Analyst (LPA) L. Hall arrived unannounced to conduct a Case Management visit. LPA met with Administrator, Oscar Vasquez and explained the reason for the visit.

While LPA L. Hall was conducting a complaint investigation 15-AS-20260122160425 on 1/29/2026. LPA was informed C1 AWOL'd on 1/26/2026. S1 stated C1 did return on 1/28/2026, to retrieve more clothing and left. S1 submitted a incident report to CCLD on 1/27/2026. LPA reviewed and requested C1's physician's report dated 1/7/2025 stating C1 is able to go into the community unassisted and reviewed the individual program plan (IPP) dated 11/17/2025 indicates C1 is able to go into the community with a friend.

Exit interview conducted and a copy of this report provided.
