Facility Evaluation Report

On 12/18/2025 at 10:10am, Licensing Program Analyst (LPA) L. Hall conducted an unannounced annual required inspection. LPA met with Licensee, Vanessa Aquino, and explained the purpose of the visit. Administrator holds a certificate #6078250740 expires 8/27/2027. The facility’s fire clearance was approved for two (2) ambulatory and four (4) non-ambulatory residents. Facility is not operating at this time.

LPA toured the facility including but not limited to bedrooms, bathrooms, kitchen, common area and back yard. The facility consists of for (4) bedrooms and two (2) bathrooms. All indoor passageways are kept free of obstruction. There are no bodies of water observed. A comfortable temperature for residents is maintained at 68 degrees F. LPA observed lighting in all rooms are adequate for the comfort and safety of the residents. Hot water temperature in the shared clients’ bathroom was measured at 105.3 degrees F. All toilets, hand washing, and bathing are safe, sanitary and in operating condition. Paper towel and soap observed at hand washing stations.

Smoke detectors/carbon monoxide were in operating condition during visit. Fire extinguisher purchased on 12/15/2025. First aid kit was observed to be complete.
