Facility Evaluation Report

On 3/12/2026 at 3:35pm, Licensing Program Analyst (LPA) L. Hall conducted an unannounced Case Management visit regarding an incident that was reported to CCLD on 3/7/2026. LPA met with Christian Smith, Direct Care Support. Justine Esomonu, Administrator arrived at 3:42pm and explained the purpose of the visit.

Upon arrival LPA observed there were four (4) clients and one (1) staff at the facility. The incident for the visit occurred on 2/27/2026, where C1 was dropped off at the Pittsburg BART station. C1 was going to visit friends for the weekend but did not return. C1 caught BART by himself. S1 stated C1 returned safe back to the facility today.

The deficiency was observed (see LIC809D) and cited from the California Code of Regulation, Title 22. Failure to correct the deficiency may result in civil penalties.
