Facility Evaluation Report

On 1/15/2026 at 10:05am, Licensing Program Analyst (LPA) L. Hall conducted an unannounced Case Management visit regarding an incident that was reported to CCLD on 1/9/2026. LPA met with Christian Smith, Direct Care Support. Justine Esomonu, Administrator arrived at 10:15am and explained the purpose of the visit.

The incident occurred on 1/8/2026, where C1 had behaviors while in the community toward staff and members of the public. C1 was taken Contra Costa Regional Medical Center and is still there as of today's date. S1 stated he was called by S2 to come to area where incident took place and the Pittsburg Police Department was contacted. S1 stated a meeting will be held today with C1's case manager to update C1's IPP.

LPA requested a copy of the IPP (Individual Program Plan) addendum dated 8/7/2025, LIC500 (Peresonnel report), LIC501 (Personnel record) for S2, and updated IPP from case manager.
