Facility Evaluation Report

On 07/21/2026 at 4:13PM, Licensing Program Analyst (LPA) Carol Fowler conducted a Case Management for deficiencies as a result of observations made during complaint investigation 15-AS-20260714144345. LPA met with Kevin Li, and explained the purpose of the Case Management.

During the course of an investigation, LPA observed the wheelchair ramps at the front door are in disrepair.

Deficiencies are cited per Title 22 California Code of Regulations and listed on LIC809D. Failure to submit proof of corrections (POC) by plan of correction due date and/or any repeat deficiencies within a 12-month period may result in civil penalties.

Exit interview conducted. A copy of this report, along with Appeal Rights, was made available to the Administrator.
