Facility Evaluation Report
On 01/16/2025 at 12:30 PM, Licensing Program Analyst (LPA) David Doidge arrived to conduct 1-Year Annual Required inspection. LPA met with Kathleen Januszewski, Program Director and explained the purpose of the visit. Day program operates from 11:00 AM to 3:00 PM, Monday to Thursday.
LPA toured facility including but not limited to: activity rooms, kitchen, bathrooms, office space, and the outside recreational area. Clients bring their own lunches and snacks are provided by the program. Emergency supplies, including water were observed. The hot water temperature in the hallway bathroom measured 106 degrees Fahrenheit. Cleaning supplies are locked and inaccessible to clients. Medications are not handled/dispensed by this program. There are no bodies of water or fire safety hazards observed. Restrooms are maintained in safe and in sanitary operating condition.
LPA reviewed five (5) client and four (4) staff files; all were complete. Emergency disaster drills are conducted every six months, with the last drill conducted 12/02/2025. Fire extinguishers throughout facility were operational last serviced 03/06/2025. First aid kit was checked and observed to be complete.
No deficiencies were cited during this inspection.
