The inspector’s account

On 07/01/2025 at 12:35PM, Licensing Program Analyst (LPA) T. Syess-Gibson arrived unannounced to conduct a case management inspection regarding facility closure. LPA met with licensee, Salina Boatner. LPA received an email dated 02/10/2025 of Licensee/Administrator advising CCL of forfeiture of license. Facility had two (2) residents at the time of notification of closure. Licensee informed residents and responsible person(s) of closure. Licensee issued a 60-Day eviction notice to the residents. The last resident moved out of the facility on 03/09/2025.

LPA toured facility including but not limited to bedrooms, bathrooms, kitchen, and outdoor areas. LPA observed that no residents were present during inspection. The Licensee/Administrator provided original license and letter of closure to LPA during visit.

LPA will send a forfeiture letter to the licensee at a later time.
