On 8/03/23 at 3:46 p.m., Licensing Program Analyst (LPA) Greg Clark arrived unannounced to deliver amended report for the allegation that staff hit and pushed resident and staff does not meet residents hygiene needs. LPA met with Administrator, Rafael Rubio Martin and explained the purpose of the visit.
Amended report delivered to administrator.
No deficiencies cited during visit. Exit interview conducted and a copy of this report provided.
