Facility Evaluation Report

On 11/21/2023 at 11:05am, Licensing Program Analyst (LPA) L. Ha ll arrived unannounced to conduct a Case Management visit. LPA met with Armando Valdez, Administrator and explained the purpose of the visit.

While LPA was conducting a complaint investigation (15-AS-20231115143225 ) on 11/21/2023. Upon arrival LPA observed construction being conducted. S1 stated that a new additional dwelling with a new address was being built on property. LPA obtain building permit during visit.

Deficiency is cited per Title 22 California Code of Regulations and listed on LIC809D. Failure to submit proof of corrections (POC) by plan of correction due date and/or any repeat deficiencies within a 12-month period may result in civil penalties.
