On 07/30/2025 at 1:50PM, Licensing Program Analyst (LPA) T. Syess-Gibson conducted an unannounced annual required inspection. LPA met with Rachel Murvine, Program Director (PD), and explained the purpose of the visit. LPA was informed of facility currently conducting an all-staff meeting. There were no staff available for annual inspection. LPA advised Program Director that LPA will return at a later date and time to conduct annual required inspection. LPA The facility’s fire clearance was approved for Four (4) Non-Ambulatory clients.
