The inspector’s account

On 07/02/2026 at 1:48PM Licensing Program Analyst (LPA) T. Syess-Gibson arrived unannounced to conduct a Case Management visit. LPA met with Caregiver, Clerazel Pike, Caregiver and explained the purpose of visit.

While LPA T. Syess-Gibson was conducting a complaint investigation (15-AS-20260629160058) on 07/02/2026. During tour of facility LPA observed facility has a lock on refrigerator in kitchen and duo doorknobs on facility’s entry door that has to be turned at the same time in order to leave the facility.

The deficiencies were observed (see LIC809D) and cited from the California Code of Regulation, Title 22. Failure to correct the deficiencies may result in civil penalties.
