Facility Evaluation Report

On 12/22/2025 at 10:45 AM, Licensing Program Analyst (LPA) James Sampair arrived unannounced to conduct the Required - 1 Year inspection. The LPA explained the purpose of the visit to Administrator (ADM) Twyla Rideout.

LPA toured the facility including but not limited to bedrooms, bathrooms, kitchen, common area, garage, and back yard. The facility consists of five (5) total bedrooms one (1) occupied by staff and three (3 ) bathrooms. All indoor passageways are kept free of obstruction. There are no bodies of water. A comfortable temperature for clients is maintained at 68.9 degrees Fahrenheit. LPA observed lighting in all rooms is adequate for the comfort and safety of the clients. The maximum hot water temperature in the kitchen was measured at 111.5 degrees Fahrenheit. All toilets were in operating condition. The supply of extra hygiene was available for residents.

Smoke / carbon monoxide detectors were in operating condition during visit. Fire extinguisher was last services on 07/14/2025. First aid kit was observed to be complete.
