The inspector’s account

On 02/03/2026, at 3:33PM Licensing Program Analyst (LPA) T. Syess-Gibson arrived unannounced to conduct a Case Management visit. LPA met with Carlos Pambid, Direct Support Staff.

While LPA T. Syess-Gibson was conducting a complaint investigation 15-AS-20260203084529 on 02/03/2026. LPA observed staff at facility had no access to client files and were unable to provide LPA with clients' files during visit. The Direct Support Staff stated administrators and house manager are the only staff members that has access to files.

The deficiencies were observed (see LIC809D) and cited from the California Code of Regulation, Title 22. Failure to correct the deficiencies may result in civil penalties.
