On 9/23/2021 LPA L.Ibo opened a case management visit. While LPA was conducting staff interview, it was found out that Administrator failed to report suspected physical abuse & S1 admitted that C1's injury was not reported to CCL office. The observed bruises and skin redness did not result to serious bodily injury.
The following deficiency was observed (see LIC 809D) and cited from the California Code of Regulations, Title 22. Failure to correct deficiency may result in Civil Penalties.
