O n 07/16/2026, at 2:31PM Licensing Program Analyst (LPA) T. Syess-Gibson conducted an unannounced Case Management visit regarding facility’s request to change from six 6 ambulatory clients to four (4) non ambulatory and two (2) ambulatory clients. LPA met with Maricris Molina, Caregiver and explained the purpose of the visit.
LPA toured the facility and observed facility has made the required changes as per the fire department’s request. Facility added a door separating the non-ambulatory rooms from ambulatory rooms.
