Facility Evaluation Report

On 02/10/2026 at 10:05 a.m., Licensing Program Analyst (LPA) L. Alexander conducted an unannounced Case Management visit regarding a report from California State Department of Development of Services (DDS) and Regional Center of the East Bay (RCEB) following a Q&A annual joint visit that was reported to the Community Care Licensing Division (CCLD) on 02/09/2026. LPA met with Direct Care Lead, Jennifer Rychlik, and explained the purpose of the visit. Program Administrator, Jesse Mendenhall, arrived approximately one hour later.

Upon entry to the facility, LPA was advised that there were three (3) clients currently at the facility and all 3 clients were in their respective bedrooms.

LPA obtained copies of the Registry of Facility Clients and Personnel Report (LIC 500).

LPA toured the facility with Ms. Rychlik, including but not limited to two (2) clients’ bedrooms, one (1) vacant bedroom, two (2) bathrooms, the living room, kitchen, and garage/storage area.
