Facility Evaluation Report

On this day, Licensing Program Analysts (LPAs) L. Ibo & L. Fici conduct case management during another visit at the facility.

While LPA’s touring the facility, at 10:29AM LPAs observed that the medication refrigerator was unlocked, there was two injectable medications inside the refrigerator which was accessible to clients in care. S3 locked the refrigerator.

Administrator moved the medication refrigerator on the staff lounge room to avoid clients’ access.

Deficiencies are cited from Title 22 California Code of Regulations and listed on 809D. Deficiencies and plan and proof of corrections were discussed.
