Facility Evaluation Report
On 10/22/2025 at 2:30PM Licensing Program Analysts (LPAs) T. Syess-Gibson and L. Hall conducted an unannounced Case Management visit regarding an incident that was reported to CCLD on 10/17/2025. LPA met with Dejah Jordan, Direct Care Supervisor and explained the purpose of the visit.
On 10/17/2025, LPA received an email from staff (S1) who notified LPA of appliances in kitchen at the facility are in disrepair. S1 stated facility's appliances have been broken over a long period of time. The following appliances are in dis-repair: microwave oven, dishwasher, stove and refrigerator.
Based on interview, observation and record review the appliances are in disrepair.
Deficiency is cited per Title 22 California Code of Regulations and listed on LIC9099D. Failure to submit proof of corrections (POC) by plan of correction due date and/or any repeat deficiencies within a 12-month period may result in civil penalties.
