Facility Evaluation Report

On 11/7/2025, at 3:15 PM, Licensing Program Analyst (LPA) James Sampair arrived unannounced to conduct a case management visit to deliver two (2) immediate exclusion letters. The LPA met with Direct Care Lead Olajumoke Omolayole and informed her of the reason for the visit.

During the visit, the LPA hand delivered the immediate exclusion letter for S1 and S2 to Direct Care Lead Olajumoke Omolayole . The LPA was informed by Supervisor Dejah Jordan over the phone that S1 had not worked at the facility and was terminated before August 2025 and that she was unsure if S2 had been terminated but that she had not worked at the facility since July 2025.
