Facility Evaluation Report
On this day, June 24, 2026, at 10:20 am, Licensing Program Analyst (LPA) Delmundo arrived unannounced to conduct a health and safety inspection as a result of the Department receiving a Priority 1 complaint (Complaint #15-AS-20260619104815). LPA met with Amanda Coray, administrator (ADM), and informed the reason for visit.
LPA toured the facility with ADM. The facility is a two-story building with courtyards on the first floor. LPA inspected the staff room, lobby/living room, kitchen, dining area, overflow supply room where including but not limited to hygiene supplies, razors, linens are kept and laundry supplies room. Custodial/mechanical, staff, medication, laundry supply and overflow supply rooms were locked when LPA arrived. LPA inspected total of 9 residents rooms and 2 bathrooms on the second floor.
No deficiency observed during today's inspection.
