Facility Evaluation Report
On 03/11/2026 at 10:40 AM , Licensing Program Analyst (LPA) L. Alexander conducted an unannounced Case Management visit regarding an incident reported to the Community Care Licensing Division (CCLD) on 02/08/2026 . LPA met with Program Administrator Amanda Coray and explained the purpose of the visit. Regional Director of Operations Michelle Porter arrived at the facility shortly thereafter.
The Department received an Unusual Incident Report (UIR) indicating that four (4) clients (C1–C4) were observed using methamphetamine and heroin inside the facility.
During the visit, LPA collected and reviewed the following documents : LIC 500, staff contact list, staff schedule (February 2026), client rosters (February 2026 – March 2026), C1–C4 admission agreements, C1–C4 physician’s reports, C1–C4 face sheets, C1–C4 emergency identification forms, C1–C4 progress notes, and additional UIR documentation dated 02/07/2026 and 02/08/2026, if applicable.

An exit interview was conducted , and a copy of this report was provided to Program Administrator Amanda Coray.
