The inspector’s account

On 2/14/2023 at 11:10AM, Licensing Program Analysts (LPAs) L. Alexander and G. Luk conducted an unannounced to conduct a case management visit. LPAs met with Administrator, Leah MacDonald.

While LPAs was at the facility for another visit, the following deficiency was observed.

At 10:45AM, LPAs observed front door had a door chain latch located near the top of the door. Staff informed LPAs that door chain latch was put on to prevent residents from leaving.

The following deficiency was observed (see LIC 809D) and cited from the California Code of Regulations, Title 22 and/or Health and Safety Code Failure to correct deficiency by POC date may result in additional Civil Penalties.

Exit interview conducted. Appeal Rights and a copy of this report provided.
