On 11/09/2022 at 12:00PM, Licensing Program Analyst (LPA) J. Sampair arrived unannounced to conduct a complaint investigation. While LPA was conducting the complaint, the following deficiencies were observed:
Unlocked cleaning supplies and bleach in kitchen and bathroom that which were accessible to clients.
Construction with no permit was being done.
The deficiencies were observed (see LIC 809-D) and cited from the California Code of Regulations, Title 22. Failure to correct the deficiencies may result in civil penalties.
Exit interview conducted. A copy of this report and appeal rights were provided.
