Facility Evaluation Report
On 07/07/2025 at 10:40 AM, Licensing Program Analyst (LPA) David Doidge arrived unannounced to conduct 1-Year Annual Required inspection. LPA met with Jason Parks, Director Day Services and explained the purpose of the visit. Day program operates from 9:00 AM to 3:00 PM Monday to Friday.
LPA toured facility including but not limited to activity rooms, kitchen, bathrooms, office space, and the outside recreational area. Clients bring their own lunches and snacks. Emergency supplies, including water were observed. The hot water temperature in the kitchen measured 110 degrees Fahrenheit. Cleaning supplies are locked and inaccessible to clients. Medications are locked in the Supervisor's office. There are no bodies of water or fire safety hazards observed. Restrooms are maintained in safe and in sanitary operating condition, and additional equipment for the physically handicapped was observed. The facility is free of odors.
Emergency disaster drills are conducted on a monthly basis at different times by each instructor. The last drill was conducted on 07/07/2025. Fire extinguishers throughout facility were last inspected 12/12/2024. First aid kit was checked and observed to be complete.
LPA reviewed five (5) participant records and six (6) staff records, and all were complete.
No deficiencies were cited during this inspection.
