On 06/30/2026 at 01:15 PM, Licensing Program Analyst (LPA) David Doidge arrived to conduct 1-Year Annual Required inspection. LPA met with Administrator Linda Kurihara states that the facility does not have any residents and is surrendering the license. .
Administrator surrendered facility license to LPA.
During the visit, LPA did not observe any resident at the facility.
No staff or resident files were reviewed.
