Facility Evaluation Report
On 05/12/2026 at 10:15AM, LPA Carol Fowler attempted to conduct an unannounced 1 year annual visit. LPA arrived at the home to find no one present. LPA left a message for a call back on the answering machine. LPA observed open blinds, an a white truck located in the driveway.
On 05/12/202 at 2:40pm, Licensing Program Analyst (LPA) Carol Fowler, conducted an unannounced annual 1-year required inspection. LPA met with Ronaldo Kasilag, Lead Care Staff LPA explained the purpose of the visit. The facility’s fire clearance was approved for four (4) ambulatory and two (2) clients.
Administrator Ron Perez arrived at 4:10pm.
LPA toured the facility including but not limited to bedrooms, bathrooms, kitchen, common area, garage, and back yard. The facility consists of four (4) bedrooms and two (2 ) bathrooms. All indoor passageways are kept free of obstruction. There are no bodies of water. A comfortable temperature for clients is maintained at 71 degrees Fahrenheit. LPA observed lighting in all rooms are adequate for the comfort and safety of the clients. Hot water temperature in the shared clients’ bathroom was measured at 110.2 degrees Fahrenheit.
