The inspector’s account

On 04/23/2025 at 11:45AM, LPA Carol Fowler attempted to conduct an unannounced 1 year annual visit. LPA arrived at the home to find no one present. LPA attempted to contact the licensee via telephone to obtain hours of operation, however, there was no answer. LPA left a message for a call back on the answering machine. LPA observed open blinds, an a white passenger van located in the driveway.

On 04/24/2025 at 9:45am, Licensing Program Analyst (LPA) Carol Fowler, conducted an unannounced annual 1-year required inspection. LPA met with Ronaldo Kasilag, Lead Care Staff LPA explained the purpose of the visit. The facility’s fire clearance was approved for four (4) ambulatory and two (2) clients.

LPA toured the facility including but not limited to bedrooms, bathrooms, kitchen, common area, garage, and back yard. The facility consists of four (4) bedrooms and two (2 ) bathrooms. All indoor passageways are kept free of obstruction. There are no bodies of water. A comfortable temperature for clients is maintained at 69 degrees Fahrenheit. LPA observed lighting in all rooms are adequate for the comfort and safety of the clients. Hot water temperature in the shared clients’ bathroom was measured at 109.1 degrees Fahrenheit. All toilets, hand washing, and bathing are safe, sanitary and in operating condition. Hand washing poster, paper towel, and soap observed at all hand washing stations. The supply of extra hygiene was available for residents. There is a minimum of 7-day non-perishables and 2-day perishables foods.
