Facility Evaluation Report

On 06/11/2025 at 9:30 AM, Licensing Program Analysts (LPAs) Y. Brown and L. Holmes conducted an unannounced annual 1-year required inspection. LPAs met with Administrator Pia Webster and explained the purpose of the visit.

The administrator currently holds a certificate (#7003531735) that expires on 02/25/2026. The facility’s fire clearance was approved for four (4) ambulatory clients. LPAs toured the facility including but not limited to bedrooms, bathrooms, kitchen, common area, garage, and back yard. The facility consists of four (4) bedrooms and two (2) bathrooms. All indoor passageways are kept free of obstruction. There are no bodies of water observed. A comfortable temperature for clients is maintained at 69 degrees Fahrenheit. LPAs observed lighting in all rooms are adequate for the comfort and safety of the clients. Hot water temperature in the shared clients’ bathroom was measured at 111.4 degrees Fahrenheit. All toilets, hand washing, and bathing are safe, sanitary and in operating condition. Hand washing poster, paper towel, and soap observed at all hand washing stations. The supply of extra hygiene was available for residents.

Smoke detectors/carbon monoxide were in operating condition during visit. Fire extinguisher was last purchased 03/24/2025. Fire drill last conducted on 06/08/2025. The emergency Disaster plan was observed and updated on 6/1/2025. First aid kit was observed to be complete.
