Facility Evaluation Report
On 09/18/2025 around 09:00 AM, Licensing Program Analyst (LPA) L. Holmes arrived announced to conduct a Case Management visit. LPA met with Latashia Perry, Licensee (S1) and explained the purpose of the visit.
While conducting file review and following up on emails, LPA received an email from Contra Costa County Fire Protection District CCCFPD advising that Licensee had not applied, paid the fees and was not ready for the inspection. LPA advised S1 that an updated STD 850 is required to ensure the safety of the Clients in care.
On 05/16/2025 a case management was conducted. Instruction from that date forward and until permitting approved, Licensee was to provide monthly communication regarding the status of the permit. A current STD 850 is required and once construction/alterations are complete, an additional updated of the fire clearance (STD850) will be required.
Deficiency is cited per Title 22 California Code of Regulations and listed on LIC809D. Failure to submit proof of correction (POC) by plan of correction due date and/or any repeat deficiencies within a 12-month period may result in civil penalties.
Civil penalty of $1000.00 will be assessed on 09/18/2025 (Fire Clearance Violation)
Exit interview conducted, LIC421M, appeal rights and a copy of this report this report provided to Latashia Perry, Licensee.
