Facility Evaluation Report

On 5/16/2025 around 09:10 AM, Licensing Program Analyst (LPA) L. Holmes arrived announced to conduct a Case Management visit. LPA met with Latashia Perry, Licensee (S1) and explained the purpose of the visit.

While conducting file review and following up on emails sent to Licensee on 10/02/24 and 02/27/25, observations, records, and interviews with Licensee revealed that the facility did not have a working landline, alterations to the existing garage were under construction, and the Administrator on record did not possess a current Standard Certificate for Administrator. LPA and S1 toured the garage/ADU and confirmed a conversion and construction has began.

On this date forward until permitting is approved, Licensee is to provide monthly communication regarding the status of the permit. Once construction/alterations are complete, an updated fire clearance (STD850) will be required.

Deficiencies are cited per Title 22 California Code of Regulations and listed on LIC809D. Failure to submit proof of corrections (POC) by plan of correction due date and/or any repeat deficiencies within a 12-month period may result in civil penalties.

Civil penalty of $500.00 will be assessed on 05/16/2025 (Fire Clearance Violation)
