Facility Evaluation Report
On 07/25/2025 around 10:30 AM, Licensing Program Analyst (LPA) L. Holmes arrived unannounced to deliver the findings for complaint #15-AS-20250530145730 dated 05/30/25 and conducted a case management for reporting requirements. LPA met with Rubidia Valenzuela, Team Lead and Mavis Freeman, CEO/Administrator (ADM) was advised of the visit.
On 05/29/2025, behavioral incidents occurred at C2’s Day Program. S1 reported the incident to C2’s Responsible Party and RCEB as required by the regulation within seven (7) days. The incident was reported to CCLD on 06/08/2025.
The deficiency was observed (see LIC809D) and cited from the California Code of Regulation, Title 22. Failure to correct the deficiencies may result in civil penalties.
