Facility Evaluation Report
On 04/30/2025 around 02:30 PM, Licensing Program Analyst (LPA) L. Holmes arrived unannounced an attempted to conduct a required annual Inspection. LPA returned on 05/13/2025 around 9:00 AM, announced and met with Rashad Fontenot, Care Staff (S1), the son of Robin Fontenot, Administrator (ADM). The facility's fire clearance is approved for three (3) clients, all ambulatory.
LPA and S1 toured facility including but not limited to three (3) bedrooms, two (2) bathrooms, kitchen, living room, dinning room, common areas and backyard. Bedrooms and living rooms were equipped with the proper furniture. Bathrooms were equipped with non-skid mats. Linens and hygiene supplies were observed inside each residents closet and there is a locked closet for medication. There is sufficient lighting throughout facility and hallways. The room temperatures were comfortable. First-aid kit was observed to be complete; never opened. Smoke detectors and carbon monoxide are combined and were operational. Fire extinguisher was observed full and needs to properly tagged. Upon receipt of new clients there shall be at least 2-days of non-perishables and 7-days of perishable foods.
