This is an official report of an unannounced visit/investigation of a complaint received in our office on

10/02/2020 and conducted by Evaluator Grace Luk

COMPLAINT CONTROL NUMBER: 15-AS-20201002152133

Resident was handled in a rough manner during COVID testing.

On 11/9/2021 at 9:45AM, Licensing Program Analyst (LPA) G. Luk arrived unannounced to conduct complaint investigation and deliver complaint findings for the allegations above. LPA met with Executive Director, Clara Allen.

During the investigation, LPA interviewed residents and staff. LPA obtained and reviewed 3 resident's files including physician's report, care plan, and emergency information.

Interview with residents indicated that staff used the shorter swab so it wouldn't be too uncomfortable. Resident did not have a bad experience with the COVID test. Interview with staff indicated that if a resident refused COVID test, then staff would stop the test.

Although the allegations may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did occur, therefore these allegations are UNSUBSTANTIATED.
