Facility Evaluation Report
On 4/30/2026 at 7:50 AM, Licensing Program Analyst (LPA) A. Gomez arrived unannounced to conduct 1-Year Annual Required inspection. LPA met with Licensee, Aurora Fe Jaquias and explained the purpose of the visit. The facility’s fire clearance was approved for 6 non-ambulatory with a hospice waiver for 4.
Upon arrival LPA observed that the facility is under renovation and having the ceilings redone. LPA observed that there is one resident on hospice at the facility who is not being affected by the renovations. LPA is unable to do a full annual evaluation at this time due to the nature of the renovations and removal of asbestos. LPA requested that the Licensee notify CCLD once the renovations are complete.
